Does LASIK Cure Presbyopia?

Presbyopia is the gradual loss of your eye’s ability to focus on close objects. It’s the reason so many patients ask me if LASIK can fix it. As an ophthalmologist serving patients in Sacramento, Roseville, and Folsom, one of the questions I hear most often is this: “I had LASIK years ago. Why do I still need reading glasses?” Or, from patients in their 40s who are noticing the blur: “Can LASIK fix my reading vision too?”

It’s one of the most common misconceptions in vision correction. And it deserves a clear, honest answer.

The short answer: LASIK with both eyes set for distance does not cure presbyopia. But there’s a LASIK-based option that can help: blended vision. One eye stays sharp for distance, and the other shifts slightly closer. And when a permanent fix makes more sense, Refractive Lens Exchange is another excellent option.

Here’s what you need to know.

What Is Presbyopia?

This gradual loss of near focus happens inside the eye, not on its surface. Most patients begin noticing it in their early-to-mid 40s. Common signs include struggling to read a menu, squinting at a phone, or holding paperwork at arm’s length.

Unlike nearsightedness or farsightedness, presbyopia is not caused by the shape of your cornea. It’s caused by the natural aging of your eye’s internal lens. That lens gradually stiffens and loses the flexibility it needs to shift focus between near and far.

This distinction matters. It’s the key reason distance LASIK cannot fully correct it.

Presbyopia affects virtually everyone over 40. By your mid-60s, the lens is almost completely rigid and the condition is fully developed. The progression is gradual, which is why patients often don’t notice it until the blurring becomes hard to ignore.

Why LASIK With Both Eyes Set for Distance Doesn’t Cure Presbyopia

LASIK (Laser-Assisted In Situ Keratomileusis) uses a precise laser to reshape the cornea, the eye’s clear outer surface. Most people picture LASIK as setting both eyes for distance. That’s highly effective for correcting Myopia (nearsightedness), Hyperopia (farsightedness), and Astigmatism.

But presbyopia is a lens problem, not a corneal problem. LASIK works on the cornea and leaves the internal lens untouched. Because of this, it cannot address the loss of flexibility that causes presbyopia. The laser cannot restore elasticity to a stiffened lens.

The same is true of PRK (photorefractive keratectomy). Like LASIK, it’s a corneal laser procedure, and it’s not the right tool for presbyopia.

Fortunately, there are proven ways to manage presbyopia surgically, starting with blended vision and extending to a permanent lens replacement.

Blended Vision: A Proactive Alternative to Distance LASIK

Blended vision, also called blended LASIK or Presby-LASIK, is a proactive alternative to distance LASIK. Rather than setting both eyes for distance, I set one eye sharply for distance. The other eye is set for an intermediate range, not full near.

This is different from monovision, where one eye is set for full distance and the other for full near. Blended vision’s smaller gap helps your eyes keep working together and preserves depth perception better than monovision.

This works best as a proactive strategy, often starting in your late 30s or early 40s. This is before presbyopia fully sets in, and the blend increases gradually as your eyes age.

It still doesn’t restore the eye’s natural lens flexibility, so it isn’t a permanent fix. It isn’t the right fit for everyone. Most patients need a few weeks to a few months for the brain to fully adjust, which is normal. I always recommend a contact lens trial simulating the blend first.

EVO ICL™ is an option if a high prescription or thin corneas rule out LASIK or PRK. It achieves the same blended-vision effect using an implantable lens instead of reshaping the cornea. On its own, it doesn’t correct presbyopia. But it keeps this strategy on the table when LASIK and PRK aren’t options.

Better Options for Presbyopia: What I Offer at California LASIK & Eye

Refractive Lens Exchange (RLE) is often an effective, comprehensive solution for presbyopia. It’s especially worth considering if you’re in your mid-40s or older.

RLE works similarly to cataract surgery. I gently remove the eye’s aging natural lens and replace it with an advanced intraocular lens (IOL). This IOL is precisely selected to match your prescription and vision goals. Unlike LASIK, RLE addresses presbyopia at its source by replacing the stiff, aging lens entirely.

RLE also carries an added benefit: it eliminates any future risk of cataracts. That’s because I replace the natural lens entirely, and the new lens can’t develop cataracts.

Who Is a Good Candidate for Presbyopia Treatment?

Not every approach is right for every patient. Here’s a general framework:

Blended vision may be a good fit if you:

  • Aren’t yet dependent on reading glasses, often in your late 30s to mid-40s
  • Still have good corneal health
  • Have already had a contact lens trial simulating the blend with acceptable results

If a high prescription or thin corneas rule out LASIK or PRK, EVO ICL can often achieve this blend instead.

RLE with premium IOLs is typically recommended if you:

  • Are 45 or older
  • Have presbyopia as your primary concern
  • Want a permanent, comprehensive solution

Previous LASIK patients who now have presbyopia aren’t necessarily candidates for blended vision enhancement. Many are excellent candidates for RLE instead.

Am I too old for LASIK?

Age alone doesn’t rule you out. In the right candidate, blended vision can meaningfully delay the need for reading glasses. Refractive Lens Exchange is specifically designed for eyes that have already aged into presbyopia. A comprehensive evaluation, not your age, determines candidacy.

Is RLE covered by insurance?

How long does RLE last?

RLE is designed to be permanent. I replace your eye’s natural lens with an artificial IOL, so the results don’t wear off.

Most patients notice a meaningful improvement within the first week after surgery. Results continue to stabilize over the following four to six weeks as your eyes heal.
